Using the distinctly British way of covering up errors as quickly as possible and hence often repeating them regularly until someone notices we miss the means of curing the problem.

In each and every one of our personal lives the greatest lessons we learn are drawn from failure and yet in business we are happy to repeat errors in order not to be viewed in the wrong light.

Failures rarely ever break the surface in an organisation where a blame culture exists. Therefore, lessons from failures are rarely shared far and wide within an organisation. And yet innovation cannot happen without failure and therefore failure is a key catalyst

Some organisations have gone as far as measuring failures and using them as a metric to understand how hard a team are trying to innovate and experiment.
